Report a Concern

If you believe a verified practice no longer meets the Locally Owned Care standard — for example, because ownership has changed, control has transferred, or the attestation was inaccurate — you can report it.

Email verify@locallyownedcare.org with the practice name and a brief description of the concern.

What Happens Next

1. We acknowledge your report within 5 business days.
2. We review the claim against the practice's verification records and public sources.
3. If the concern appears substantiated, the practice is contacted for a response.
4. A determination is made within 30 days of acknowledgment.

If Verification Is Revoked

The practice's registry listing is updated immediately. Digital badges must be removed within 48 hours. Physical signage must be removed within 14 days.

Confidentiality

Reports are reviewed internally. Reporter identity is not shared with the practice unless required by law.
