What We Collect
When a practice applies for verification, we collect the practice name, location, owner name(s), ownership percentage, and supporting documentation (such as a Schedule K-1). This information is submitted voluntarily by the practice owner via email.
We do not collect personal information from visitors browsing the site.
How We Use It
Submitted information is used solely to evaluate verification eligibility. If approved, the practice name, location, type, owner name(s), and ownership percentage are displayed in the public registry.
Supporting documents (K-1s, operating agreements) are reviewed during the verification process and are never published or shared.
Analytics
This site uses GoatCounter, a privacy-friendly analytics tool. GoatCounter does not use cookies, does not collect personal data, and does not track visitors across sites. It records only aggregate page view counts.
Document Submission
Applicants submit ownership documents via email at their own discretion. Locally Owned Care does not provide a secure upload portal. By submitting documents, applicants accept the inherent risks of email transmission.
Data Retention
Verification records are retained as long as the practice maintains active verification. If verification ends, the practice is removed from the public registry. Supporting documents are deleted within 30 days of verification ending.
